The Integrated Home and Community Support Service within Older Persons Health on the West Coast is the place to be if you’re keen to make a difference in our community. Our service is forward thinking and we are one of the teams in New Zealand using InterRAI to effectively assess the needs of our patients.
Your combined nursing and case management skills will ensure clients receive necessary supports and intervention to remain in their home. As a key member of the multidisciplinary team, you’ll case manage a load of patients, working In conjunction with their GP’s and home based support services, to ensure patients receive appropriate and timely care to prevent hospital admissions.
Your passion and experience in gerontology, community and/or primary health nursing will see you successfully; complete comprehensive clinical assessments using InterRAI, utilise your exceptional critical thinking skills and manage the care of a complex case load of patients.
Excellent interpersonal, organisational and computer skills, sound clinical knowledge of the ageing process, the ability to positively engage support staff and provide sound clinical leadership are imperative to this role.

This is a .8 role covering four days per week, with which days negotiable.
