Our Complex Clinical Care Network (CCCN) is looking for a flexible and motivated team member to work within the Clinical Assessment (CA) team for a fixed-term period of 12 months.
Your role will be to assess clients in a variety of health settings using the interRAI Home Care assessment tool and create a plan of care that is client centred, restorative and goal focused including developing support packages in a holistic and culturally appropriate manner for older people who require long term support in the community.
Ideally you’ll have experience working with older people in a hospital, community or aged care environment, and be confident working with them and their family/whanau to achieve the best outcomes. To be successful in this role you’ll already have excellent knowledge of community resources and services that are available to the older person.
Your verbal and written communication skill level is high, you’ll be experienced working with electronic systems and be confident using a computer. Previous experience with the use of interRAI would be an advantage, but not essential as extensive training and on-going support in its use is provided.
You must be a Registered Health Professional with a current annual practising certificate who has excellent interviewing, assessment and problem solving skills, and a full, clean NZ driver’s licence.
